Position : Adult Case Manager – West
Job Description : GENERAL DESCRIPTION: This is a professional position providing a wide range of advocacy and case management services to adults with severe and persistent mental illness. The successful candidate will need to function with a great deal of professional independence under the general direction of the Adult Services Division Director.RESPONSIBILITIES AND DUTIES: Provides advocacy, linkage, coordination and treatment interventions to adult and families engaged in treatmentProvides all necessary outreach services to maintain identified consumer’s stability in the community and medication complianceProvides Person Centered Treatment Planning and Service DeliveryDocuments all clinical activities in accordance to established standardsAbility to meet agency productivity standardsMINIMUM QUALIFICATIONS: Bachelors degree in Social Work, Psychology, or related degree that is Medicaid Billable from an accredited schoolAppropriate State of Michigan licensure requiredExperience working with the Severe and Persistent Mentally Ill populationValid MI license and ability to meet agency’s insurance company’s standards for coverageSALARY: Commensurate with Experience (Base salary, plus 20% fringe)Job Type: Full-timePay: $35,438.00 – $48,724.00 per yearBenefits:Dental insuranceEmployee assistance programEmployee discountFlexible spending accountHealth insuranceHealth savings accountLife insurancePaid time offProfessional development assistanceRetirement planVision insuranceSchedule:Monday to FridaySupplemental Pay:Bonus payCOVID-19 considerations:Regular sanitization, staff and consumers must pass COVID screenings, PPE required and provided.Education:Bachelor’s (Preferred)Experience:Case management: 1 year (Preferred)License/Certification:Driver’s License (Preferred)State of Michigan licensure (Preferred)Work Location: One location
